Contact Your Local Police Department
If you've been the victim of identity theft or any other form of fraud it is important that you contact your local police department and file a report. If is also a good practice to request a report number and a copy of the report for your records.

Report a Lost/Stolen Check Card
To report a lost/stolen check card please call 1-800-528-2273.
Notifying Credit Bureaus
If you've been the victim of identity theft it is highly recommended that you contact the three national credit bureaus. Make sure to request that each credit bureau place a "fraud alert" on your credit report. It is also a good practice to request a copy of your credit report when reporting identify theft.
